Hours Change Request
Submit this form if your hours aren’t showing up correctly or if you’re missing an entry not due to a sign out failure.
A few examples are incorrect hours modifier (missing double hours on holidays), shift cancelled due to weather, someone accidentally signed you out early, or you asked someone to sign you out after you left.
Forgot to Sign Out
Submit this form if you signed in but forgot to sign out, or if the system did not allow you to sign out.
